If you order a fish dish, for example, pasta with fish or seafood, don't ask for Parmigiano Reggiano. Many Italians find the idea unthinkable.
Many of the larger churches, e.g. cathedrals in Italy, may ask you to cover bare shoulders or legs if your skirt/shorts are too short.
While you will hear 'ciao' a lot in Italy, it's really best to use it with people you know. Instead, greet strangers 'buongiorno/buonasera' or 'salve'.
Aside from larger cities like Rome or Milan, many other cities will close around 12.30/1pm for lunch. They may not open till 3.30/4pm.
The majority of Italian breakfasts are sweet rather than savory. So unless you dine at your hotel, expect a pastry over eggs and bacon.
